- comments: observed evidence of pest activity on the premises. Observed approx. 25 rodent (mice) droppings on the floor in the corners under and around the storage racks at rear dry storage rooms and behind the hot water heater next to the dishwashing 3-compartment sink area,and inside of an unscreened east wall vent behind the kitchen coolers. Additional pest control service is needed to eliminate the pest activity. Must detail clean and remove all rodent (mice) droppings from all affected areas and maintain. Priority foundation violation 7-38-020(a),no citation issued.

- comments: observed gap/opening at bottom right corner of east side exit door #2. Instructed to make door tight fitting all across the bottoms and corners to prevent pest entry and maintain.

- comments: observed stained cutting boards stored on kitchen storage rack. Instructed to resurface or replace the cutting boards and maintain.

- comments: observed water leak drain pipe underneath the 1 comp prep sink located at west wall near the window areas. Instructed to repair plumbing leak and maintain.

- comments: instructed to detail clean and organize the rear storage room and remove unnecessary articles due to potential pest harborage and remaining items must be properly stored and elevated off the floor at least 6' inches to facilitate cleaning.
