- comments: observed 15-20 mice droppings underneath front cabinet next to hand washing sink. Instructed manager to contact pest control company for service, remove droppings, clean and sanitize. Priority foundation 7-38-020(a). Citation issued

- comments: observed cutting board on main cook line covered with deep grooves and dark stains. Instructed to repair or replace and maintain smooth, cleanable surfaces. Manager removed cutting boards,washed, rinsed and sanitized, flipped them over and returned to prep area.

- comments: observed no covered waste receptacle for feminine hygiene products in toilet room. Instructed manager to provide.

- comments: observed floors in rear storage area in need of cleaning. Instructed manager to detail clean and maintain.
