- comments: 2-102. 14 observed no person in charge with valid chicago food sanitation certificate. Instructed manager to designate person in charge during all hours of operation.
2. City of chicago food service sanitation certificate
- comments: 2-101. 11 observed no certified food manager on duty upon arrival while tcs foods are being prepared,handled and served such as (sushi) must be on site at all times. (priority foundation 7-38-012)(citation issued)
10. Adequate handwashing sinks properly supplied and accessible
- comments: 5-202. 12 (a) observed hot water reaching a maximum temperature of 82f at customer washbowl sinks. Instructed manager to ensure all hand washing sinks reach a minimum temperature of 100f. Violation previously mentioned in report #2560737 on 7-14-2022 priority violation 7-38-030(c). Citation issued.
38. Insects, rodents, & animals not present
- comments: observed several dead roach and other insect carcasses throughout kitchen including on walls, behind equipment and on interior of paper towel dispenser. Instructed manager to remove carcasses and sanitize areas.
39. Contamination prevented during food preparation, storage & display
- comments: 3-307. 11 observed serveral boxes of food stored on floor. Instructed manager ensure all food is at least 6 inches above the floor.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: 4-501. 11 observed inadequate number of drain plugs for 3-comparment sink. Instructed manager to provide at least 3 drain plugs for 3-compartment sink.
55. Physical facilities installed, maintained & clean
- comments: 6-501. 114 observed heavy clutter in kitchen area. Instructed manager to de-clutter floor and counters in kitchen area.
57. All food employees have food handler training
- comments: 2-102. 13 observed employees handling food with no food handler certification. Instructed manager to ensure all employees have food handler certification.
58. Allergen training as required
- comments: 2-102. 13 observed no certified food manager on site with food allergen training. Instructed manager to ensure there is a certified food manager on site with food allergen training at all times.
59. Previous priority foundation violation corrected
- comments: previous priority foundation violation not corrected within specified timeframe. Report #2560737 dated 7-14-2022 found small fly and roach activity which was to be corrected by 7-21-2022. Observed 2 roaches in kitchen prep area and approximately 40 small flies in prep area. Priority violation 7-42-090. Citation issued
