- comments: observed no hand washing sink in front serving/coffee prep area. Instructed manager to install hand washing sink with hot and cold running water, soap and paper towels in front serving area. Priority foundation 7-38-030(c)

- comments: observed bulk containers not labeled in prep and storage areas. Instructed manager to label all bulk containers with common name of food.

- comments: observed over 10 rat droppings on floor along walls near oven in first floor prep area, 10 rat droppings on floor along bottom stairway leading to basement, 10 rat droppings on floor along east wall behind chest freezers in basement, 20 rat droppings on floor along west wall near stand mixer and chest freezers, 10 rat droppings on floor ner 2-compartment sink in basement, 30 rat droppings on floor under shelving units in single service article storage in basement. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued.

- comments: observed food handlers not wearing hair restraints. All food handlers must wear effective hair restraints.

- comments: observed soiled wiping cloths on prep table. Instructed manager to store in sanitizing solution between uses.

- comments: observed single service articles not inverted and service area and stored on floor in dining, prep, and storage areas. Instructed manager to invert and elevate all single service articles off floor.

- comments: observed raw wood shelving in basement single service article storage area. Instructed manager to seal all raw wood surfaces. Shelving must be smooth and easily cleanable.

Instructed manager to clean and maintain all food and non-food contact surfaces and equipment.

- comments: observed leaking faucet on 3-compartment sink. Instructed manager to repair and maintain.

- comments: observed accumulated food debris, grease buildup, dust buildup, and dead roaches on floors, under all equipment, along walls and in all corners in prep, storage and basement areas. Observed dust buildup on ceiling vents and ceiling fans in all areas. Instructed manager to clean and maintain.

- comments: observed excessive clutter in dining, front serving, prep and basement areas. Instructed manager to remove all unnecessary articles and equipment to prevent pest harborage.

- comments: observed missing light shields in first floor and basement prep areas. Instructed manager to replace and maintain.
