- comments: found no handwashing signage at rear food preparation area. Must provide.

- comments: found no discard date for the following ready to eat (rte), time/temperature control for safety foods (tcs) held for over 24 hours: cooked chorizo and cooked tuna. Instructed manager to provide discard date for all rte, tcs food held for over 24 hours. Priority foundation violation. 7-38-005. Citation issued.

- comments: observed eggs and burgers on menu said items can be ordered under cooked. Instructed facility to provide footnote, disclosure and reminder for all menu items that can be ordered under cooked. Priority foundation violation. 7-38-005. No citation issued.

- comments: found approx 80 - 100 dead roaches in the following areas: behind front food preparation under beverage station along entire wall (300), behind coolers in 1st rear food preparation area along entire wall(30), also noted 30 in 2nd rear food preparation area along entire wall (30), under stair case in basement(10). Must remove, clean sanitize and maintain.
