Chicago, 60641 Illinois, United States

Inspectors found several significant food safety violations during the inspection. Handwashing sinks in the prep/dishwashing area lacked soap and paper towels, though soap was provided during the inspection. Inspectors also found spoiled chorizo and sausage with mold in a walk-in cooler, and several foods, including chicken, chorizo, sausage, and beef, were held above the required cold temperature. The operator voluntarily discarded the affected foods. The walk-in cooler was not maintaining the required temperature, and a cooler was tagged as held for inspection until rechecked by the health department.

Inspectors also reported pest activity, including more than 50 dead roaches in the rear prep area, about five live roaches in prep and dry storage areas, and a decomposing rat in the basement storage area. Additional issues included a non-food-grade sprayer being used on potatoes, food and grease buildup on equipment and surfaces, a dirty meat slicer and fryer filtering unit, cardboard used as shelf liners, wiping cloths not stored in sanitizer, liquid pooling on floors, and heavy dirt, grease, and food debris on floors and walls. Citations were issued for multiple significant violations.
