- comments: observed no hand washing sings or posters near hand washing sink in food preparation area. Must provide and maintain.

- comments: observed black and brown slime substance on interior water components of the ice machine dripping into ice. Ice is used for human consumption. Instructed management to turn off machine remove ice and have interior of ice machine cleaned and sanitized and maintain. Priority foundation violation 7-38-005, citation issued.

- comments: observed ready to eat tcs foods held more than 24 hours (cooked noodles, beef soup, cooked pork and steam rice). Not date marked to indicate the date in which the food must be consumed or discarded. Instructed manager to date mark all ready-to-eat tcs foods held more than 24 hours. Priority foundation violation 7-38-005, citation issued

- comments: observed all food handlers not wearing hair restraints while food is being prepared in main kitchen food prep area instructed manager all food handlers must wear effective hair restraints at all times food is being prepared and served.

- comments: observed wiping cloths stored on top of prep tables and 3compartment sink. Must properly store wiping cloths in a sanitizing solution when not in use.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no quaternary chemical test strips on site for the quaternary sanitizer being used at the three compartment sink. Must provide quaternary chemical test strips to monitor sanitizing levels. Priority foundation violation 7-38-005 citation issued.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacle in the unisex employee washroom. Must provide covered waste receptacle and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed grease and dirt build up on floor under around and behind all cooking equipment. Must clean and maintain.
