- comments: 7-206. 12 observed mulitple bags of rodent bait and rodent poison bait blocks stored on the floor throughout the basement. Management instructed that all rodent poison and bait must be stored in a covered, tamper-resitant bait staion. Priority violation 7-38-015. Citation issued.

- comments: 6-501. 112 remove the dead rodent from the basement floor.

- comments: 6-501. 11 replace the broken floor tiles in front of the 3 compartment sink behind the bar. The wooden baseboard behind the bar is detaching from the wall. Management instructed to repair. Seal all wall openings in the basement behind the storage shelves and the wall next to the rear exit door.

- comments: 6-501. 114 remove the clutter between the wall and the walk-in cooler and between the wall and basement staircase to eliminate pest harborage conditions.

- comments: 6-202. 11 observed end caps missing from the light shields above the food prep area. Management instructed to install end caps where missing.
59. Previous priority foundation violation corrected
- comments: 8-404. 13(b)(3) previous priority foundation violation from report #2570212 on 1/25/23 not corrected: 38 - observed approx. 75 rat droppings on the floor along the walls and under the shelves and equipment in the basement storage areas. Must clean and remove all rodent droppings from the affected areas. Additional pest control service is needed to eliminate the pest activity. Priority violation 7-42-090. Citation issued.
