- comments: observed no written employee health policy on the premises. Management instructed to provide a signed employee health policy for each employee. Priority foundation 7-38-010.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean-up of vomiting and diarrhea clean-up kit on premises during inspection. Instructed to have required equipment for clean-up. Priority foundation 7-38-005.
37. Food properly labeled; original container
- comments: observed several types of prepackaged cakes and bread in the retail display cooler not properly labeled. Management instructed to label all prepackaged foods for sale.
38. Insects, rodents, & animals not present
- comments: observed an infestation of live cockroaches in both food prep/utensil washing areas. Observed approximately 50 live cockroaches on the storage rack holding the bags of flour and sugar, on the ceiling in this area, and on the walls and ceiling and pipes in the basement food prep and utensil washing area and on the walls of the bakery prep area. Also observed 10 live small flying insects in the basement food prep area. Also observed approximately 50 mouse droppings on the floor and rails inside of the large bakery oven. Management instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity throughout the premises. Priority 7-38-020(a). Citation issued.

- comments: observed dead cockroaches on glueboards in the food prep and storage areas and on floors and boxes of stock. Management instructed to remove and clean all dead cockroaches from the premises.
40. Personal cleanliness
- comments: observed a food handling employee not wearing a hair restraint while preparing food in the food prep area. Management instructed that all food handling employees must wear a hair restraint in the food prep area.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed open painted crevices along the sides of the basement grease trap. Must paint and seal the grease trap to prevent pest harborage conditions. Replace the worn conveyor belt for the dough sheeter.
49. Non-food/food contact surfaces clean
- comments: clean the storage shelf beneath the wood prep table in the 1st floor bakery prep area. Clean all bulk food storage containers. Clean the interior of the prep coolers. Remove all cardboard flats with cockroach droppings and debris and discard them. Clean the flour debris from the dough sheeter.
51. Plumbing installed; proper backflow devices
- comments: must install a backflow prevention device on the mop sink faucet.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: must adjust the bathroom door so that the self-closing door device works properly.

- comments: seal the openings in the ceiling of the basement food prep area above the 3 compartment sink.
56. Adequate ventilation & lighting; designated areas used
- comments: must install a working ventilation fan in the washroom. Must move the stove underneath the ventilation hood.
