- comments: observed bare hand contact of ready to eat tcs foods by food handlers such as male employee removed ready to eat turkey from sandwich with his hands put it on the grill and removed with bare hands again. Another food handler removed cooked bacon from grill with bare hands and placed on plate to be served. Must use gloves,utensils or deli paper for handling ready to eat foods. - comments: observed consumer advisory of consuming raw and undercooked foods on menu reminder marked incorrectly on menu items must update and maintain. (priority foundation 7-38-005)

- comments: must label food storage containers when food is not in original package (squeeze bottles ,shakers, food containers)

- comments: observed evidence of an infestation of appx. 300 or more mice droppings scattered throughout premises such as on floor along wall bases of dish machine and scrape sink,on several shelving units and counter top & on floor along wall bases behind chest freezers in employees area. Rear storage area on shelving units and on floor in between clutter on floor. Underneath bags stored on floor in corner near rear door. Dry food storage area -on floor under shelving units, on ledge of ladder,behind buckets. Front service area- scattered on shelving units, under ice cream freezer on counter top,lower shelf underneath pick up window. Front service counter- on shelving units. Dining area-under booth seats on interior ledges and where food is stored on tables. Men's toilet room -on floor around toilet. Women's toilet room-on floor behind garbage can,along wall bases in both stalls. Must remove droppings clean and sanitize areas. Must have a pest control operator to service premises on or after 5/15/23. (priority 7-38-020(a)(citation issued)

- comments: observed a gap along bottom of rear door and front door in center must make doors tight fitting to prevent rodent entry.

- comments: must store wiping cloths in a sanitizing solution and not on surfaces and tables when not in use.

Delime dish machine.
