- comments: 4-201. 11 observed a cold holding unit across from the ventilation hood with two sliding doors not properly closing to cover the top food items in the unit. Instructed to repair or replace said cold holding unit.

- comments: i observed the establishment's grease receptacle open, exposing an accumulation of food inside the grease receptacle. I also observed grease and food residue on the exterior of the grease receptacle. Instructed to keep the lid on the grease receptacle closed at all times, clean the exterior of the grease receptacle, and advised manager to contact company to empty and clean said grease receptacle. Priority foundation violation 7-38-020(b), citation issued.

- comments: observed a certified food manager on duty with no allergen training certificate. Instructed all certified food managers to provide and maintain records on-site at all times.
