- comments: observed employee washing rinsing multi use utensils at three compartment sink and store them. No sanitizer solution used at three compartment sink. Instructed to provided sanitizer at third compartment sink. Manager provide chlorine sanitizer,100 ppm priority violation:7-38-005,citation issued

- comments: must provide proper labels for date marking the preparation and consume by/discard date (maximum of 7 days total) of all refrigerated, ready-to-eat, tcs foods prepared on site and held over 24hrs. Priority foundation violation#: 7-38-005. No citation issued.

- comments: must indicate on menu 'cook to order' phrase for eggs. Priority foundation violation:7-38-005, no citation issued

- comments: need to label food containers with common name at second floor food storage area.

- comments: observed rodent droppings in second floor storage area, over 50 mice droppings under shelves, corners, and along baseboard wall. We observed small flies under a three-compartment sink anf around a grease trap in the kitchen. Remove droppings and flies, clean and sanitize area. Priority foundation violation: 7-38-020(a), citation issued

- comments: all wiping cloths in all prep areas must be held in a container with a sanitizing solution between use.

- comments: knives stored between the wall and electrical tubing by the deep fryer,must relocated to a knife rack after washed,rinsed and sanitized.
