- comments: observed slight calcium build-up inside ice machine. Must clean interior of ice machine and maintain.

- comments: observed 5 live roaches in prep area walls/cracks and crevices. Instructed manager must remove all roach activity by the date of reinspection. Advised to call pest control operator for service. Priority foundation 7-38-020(a).

- comments: observed accumulated food debris inside coolers door rubber gaskets and exterior of all coolers and prep area shelves. Must clean coolers door gaskets and all equipment.
55. Physical facilities installed, maintained & clean
- comments: observed accumulated food/grease and dirt build up on prep area floor along walls/corners and under cooking equipment. Must detail clean floor throughout prep area and maintain.
