- comments: observed no signed employees health policy. Must provide and maintain. (priority foundation 7-38-010)(no citation issued)

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean-up policy procedure and items for vomiting and diarrhea. Must provide and maintain. (priority foundation 7-38-005)(no citation issued)

38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 150 or more mice droppings scattered on & under display shelves at chip display,pop,cereal,can goods,dog food,candy,photo shop,front cashier area & liquor,rear liquor area,staff break room near lockers,make-up ,rear storage area -on rolling carts,on floor,on bins,in between crates. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 2/3/21. (priority foundation 7-38-020 (a)(citation issued)

- comments: observed an appx. '1/2-3/4' gap along bottom of both entry /exit doors. Must make doors tight fitting.

- comments: must clean debris build up from walk in cooler-shelving units,fan guard covers.

- comments: must repair or replace stopped up mopsink filled with dirty waster and staff toilet room cold water faucet in poor repair at 1st sink.
