- comments: observed no signed employees health policies. Must provide and maintain. (priority foundation 7-38-010)

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean-up policy procedure and items for vomiting and diarrhea. Must provide and maintain. (priority foundation 7-38-005)

36. Thermometers provided & accurate
- comments: observed no probe thermometer for taking food temperatures. Must provide and maintain. (priority foundation 7-38-005)

38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 40 or more rat droppings on floor in between unused reach in coolers in prep area,in between clutter on floor near rear door and under and on top of pallets in rear storage area,under storage shelves on sales floor. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 6/10/20. (priority foundation 7-38-020(a)

- comments: observed a large brown cat running around in store. Also. Open cat food in a styrofoam bowl in rear storage area. Cat feces on floor under mop sink. Cat was chased from inside of store during inspection,cat food and feces was removed from premises. Must clean and sanitize areas. No animals allowed on premises unless it is a service animal. (priority foundation )

- comments: observed all male employees wearing open toe sandals in prep areas. Must provide closed toe shoes.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must remove cardboard lined on top of prep tables under table top deep fryers.

49. Non-food/food contact surfaces clean
- comments: must clean debris build up from meat slicer,saw machine,prep tables,reach in coolers and unused coolers,3- comp basin. Must defrost all chest freezers.

55. Physical facilities installed, maintained & clean
- comments: must provide smooth and easily cleanable floor surface in front prep area. Must repair or replace damaged and missing floor tiles throughout premises,ceiling in poor repair on sales floor and in rear prep area,leaky ceiling in rear storage area.

- comments: observed wall mounted residential ventilation system in rear prep area used for 3- table top deep fryers. Must install proper ventilation unit.
