22. Proper cold holding temperatures
- comments: found the following tcs foods stored inside the rear walk-in cooler (wic) at improper temperatures: ham, chicken, turkey, soup, salami, cheese, sliced tomatoes, meatballs, tuna, roast beef, pepperoni, cold cut combo and steak were at temperatures ranging from 45. 5f to 49. 8f, a combined weight of approximately 200lbs. Valued at $2,000. All items were discarded of by pic. Instructed to keep cold tcs foods at 41f or lower. Maintain same. Priortiy 7-38-005 citation issued

33. Proper cooling methods used; adequate equipment for temperature control
- comments: found the rear walk in cooler not able to hold the required temperature of 41f or lower. Ambient temperature was 49. 0f per metal stem thermometer and thermometer on the outside of the cooler. Instructed to have unit serviced so that it maintains an ambient temperature of 41f or lower. Priority 7-38-005 citation issued

38. Insects, rodents, & animals not present
- comments: found a fly strip hanging in the rear prep area. Instructed to remove fly strip and refrain from hanging strips in prep areas. Maintain same.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: found the rear slicer's attached cutting board with deep, dark grooves and stains. Instructed to sand or replace cutting board to maintain a smooth and easily cleanable surface.

51. Plumbing installed; proper backflow devices
- comments: found the front hand-washing sink leaking from the pipe underneath it. Instructed to fix leak and maintain good plumbing.

52. Sewage & waste water properly disposed
- comments: found a tube draining into a plastic container on the top shelf inside the walk in cooler. Instructed to rerout tube to a floor drain and maintain same.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: found both toilet rooms in need of cleaning. Instructed to clean and maintain toilets.

55. Physical facilities installed, maintained & clean
- comments: found mop in the rear not properly stored when not in use. Instructed to store hanging up for proper air drying.

- comments: found the core violations from report #2627760 dated 12/01/2025 not corrected and/or maintained. Instructed to correct and maintain: -38. Observed the front entry door with about a 1/4 inch gap down the middle and a hole in the door where the handle is missing (door with the open sign) instructed to seal and repair as needed as a means of preventing possible pest activity. -47. Observed the two lower reach under the merrychef toasting machine in need of a detail cleaning from the build up of dirt. Instructed to clean and maintain. -47. Observed the floor drain under the three compartment sink in the rear with a heavy build up of some sort of black substance. Instructed to clean and maintain. -49. Observed the floor of the walk in cooling unit in need of a detail cleaning. Instructed to clean and maintain. -55. Observed the floor in the rear dish washing area under the three compartment sink and the shelving area in the rear as well as under and around the equipment in the front food service area in need of a detail cleaning. Instructed to clean and maintain all areas. Priority foundation 7-42-090 citation issued
