2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty while tcs foods (steak, chicken) were prepared and served at the time of inspection. Certified food manager arrived at facility 35 minutes after inspection started.
Instructed person in charge that a certified manager with a current city of chicago certificate must be on premises at all times when time and temperature control for safety(tcs) foods are being prepared, handled and/or served. Priority foundation violation #7-38-012. Citation issued. Cos
3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain with verifiable signed copies on site for all food employees. Priority foundation violation #7-38-010. No citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed handsink in the kitchen food prep area not stocked with hand soap. Informed person in charge that hand soap must be available at handsinks at all times. Food handler obtained hand soap for hand sink during time of inspection. Priority foundation violation #7-38-030(c). Citation issued. Cos

- comments: observed no handwashing signage at the hand sink in the kitchen food prep area and bar area. Instructed to provide and maintain.

- comments: observed refrigerated tcs foods not labeled or dated. Instructed manager refrigerated, ready-to-eat, tcs food, prepared on site and held greater than 24 hours must be labeled with sell by date included. Priority foundation 7-38-005. Citation issued. 35. Approved thawing methods used
- comments: observed facility not meeting thawing requirements. Food handler thawing frozen pork on the food prep table. Informed food handler of proper thawing methods ( must utilize running water with enough volatility when thawing). Must meet proper thawing requirements and maintain.
38. Insects, rodents, & animals not present
- comments: observed a 1/4' gap at the bottom right of the rear exit door. Must provide a tight fitting seal at all outer openings to prevent the entry of pests.
41. Wiping cloths: properly used & stored
- comments: observed wet wiping cloth hanging on the side of the three compartment sink and laying on the prep table in the kitchen. Must properly store wiping cloths in a sanitizing solution between use.
55. Physical facilities installed, maintained & clean
- comments: observed missing ceilings tile panels in the women toliet room. Instructed to replace the missing ceiling tiles.
57. All food employees have food handler training
- comments: observed employees on site with no food handler certificates. Instructed to provide and maintain records at all time
58. Allergen training as required
- comments: observed the certified food manager without an allergen training certificate. Management instructed to have all certified food managers obtain this certificate.
60. Previous core violation corrected
- comments: observed previous core violation #47(observed plastic grocery bags being used for food storage in freezer. Must use food grade containers that are easily cleanable) not corrected within 90 day time frame on the inspection report dated for 10/13/20. Instructed person in charge that the violation shall now classified as a priority foundation item due to non previous noncompliance. Priority foundation 7-42-090 no citation issued.
