- comments: observed cold tcs foods, held at improper temperatures ranging between 46. 8-55. 9f. Observed cold tcs foods, chicken taquito(48. 7f), shredded cheese(50. 9-53. 2f), diced tomatoes(49. 2f), rice(50. 5-52. 0f),cheese&potato flauta(50. 0f), shrimp(53. 2f), chilaquiles with cheese(50. 4f), heavy cream(52. 2f), sour cream(46. 8-47. 8f), pork(48. 2-55. 9f), cooked vegetables containing potatoes(48. 9f), fish ceviche(50. 2f), birria au jus(47. 5f), garlic bread with butter and cheese(52. 5f), beef(52. 7f), salmon(54. 7f)and octopus(54. 0f) held at improper temperature. Instructed all cold tcs food must be held at 41f or below. Corrected on site manager voluntarily discarded and denatured food valued at $500 weighing 70lbs priority violation 7-38-005 citation issued

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed two door refrigerator in first floor rear prep area at improper ambient temperature of 52. 0f. Instructed all cold holding units must be maintained at 41f or below. Corrected on site held for inspection tag placed on two door refrigerator in first floor prep area. Priority violation 7-38-005 citation issued

38. Insects, rodents, & animals not present
- comments: observed opening at lower left corner at base of rear exit door leading to dumpster area. Instructed to seal all outer openings to prevent pest entrance.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed rust on shelving units in both areas of outside walk in cooler. Instructed to remove rust from all shelving units in outside walk in cooler nonfood contact surfaces of equipment shall be constructed of a corrosion resistant, nonabsorbent and smooth material.

51. Plumbing installed; proper backflow devices
- comments: observed water line leading from ice machine draining onto floor underneath three compartment sink in basement. Instructed to repair water line leading from ice machine for proper drainage into floor drain. Observed leaking drainpipe underneath wash compartment of three compartment sink in basement dishwashing area. Instructed to repair drainpipe in said area and maintain.

52. Sewage & waste water properly disposed
- comments: unable to verify existence of grease trap on two three compartment sinks in basement. Instructed to install grease traps on both three compartment sinks in basement so that they shall be located to be easily accessible for cleaning.

56. Adequate ventilation & lighting; designated areas used
- comments: observed flat top grill in basement prep area not completely installed under ventilation system. Instructed to install flat top grill in basement prep area completely under ventilation system or remove from premises.

58. Allergen training as required
- comments: at time of inspection facility provided no allergen awareness training certificate for one employee with city of chicago food service sanitation certificate. Instructed to provide proof of allergen training for all employees with city of chicago food service sanitation certificates.
