- comments: observed no written employee health policy on the premises. Management instructed to have a signed employee health policy for each employee. Priority foundation 7-38-010. No. Citation issued

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean up procedure and supplies on the premises to respond to vomit and diarrheal events. Management instructed to provide both. Priority foundation 7-38-005. No citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hot running water at front hand washing sink in food preparation area and rear dishwashiing area. Instructed manager to provide at least 100f water at hand washing sink for proper hand washing. Employee instructed to use hand sink in front prep area for hand washing. Priority violation 7-38-030(c)citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand soap at the hand washing sink located next to 3-compartment sink in rear dish washing area. Instructed to provide soap at all times. Person in charge provided soap during inspection. Priority foundation violation. 7-38-030(c). Citation issued.

- comments: observed low temperature dish machine in rear prep area not properly sanitizing. Sanitizing strength of sanitizer (chlorine) at the time of inspection was at 00 ppm. Instructed to use 3-compartment sink to wash, rinse and sanitize dishes and to not use dish machine until equipment is properly sanitizing at a minimum of 50 ppm. Dish machine is tagged until notified by cdph for re-inspection. Priority violation. 7-38-025. Citation issued

- comments: observed improper cold holding temperatures of tcs foods inside of the walk-in cooler and top of food prep tables. Observed cooked potatoes at 59. 9. 7f, stew at 59. 3. 5f, cooked chicken 69. 6f, macroni cheese 56. 3f, grill chicken 62. 2f and cheese 58. 6f management immediately and voluntarily discarded approximately 45# of food worth $80. Priority 7-38-005. Citation issued.

- comments: observed ready-to-eat, tcs foods held over 24 hours (green beans,rice cooked chicken, soup ) not date marked to indicate the date in which the food must be discarded or consumed. Management instructed to date mark all ready-to-eat, tcs foods held more than 24 hours. Manager discarded #30 of food worth $80 priority foundation 7-38-005. Citation issued. -

- comments: observed approximately 35 live small flies on wall, ceiling and food storage shelving in kitchen food preparation area. Instructed to clean and sanitized affected areas. Additional pest control service is needed to eliminate flyong insect problem. Priority foundation violation 7-38-012(a). Citation issued.

- comments: outer openings not protected side door with gap between door and frame. Must repair and maintain.

- comments: observed employees cooking and preparing food without wearing hair restraints. Instructed that all employees must wear hair restraint at all times food is being prepared and served.

- comments: observed wiping cloths stored on top of prep tables. Must properly store wiping cloths in a sanitizing solution when not in sue.

- comments: observed ice scoops stored on top of ice machine in front bar area. Must properly store ice scoops in a clean container to prevent contamination.

- comments: observed broken rubber gasket at the 1door prep cooler in kitchen food preparation area. Must repair and maintain.

- comments: observed dirt and grease encrusted on sides, inside and top of all the cooking equipment throughout including the ovens, hot holding unit, chest freezer, deep fryer, storage shelves, prep coolers, prep tables, 3 compartment sink and hand washing sinks in rear food prep area. Must clean all equiment and maintain.
