- comments: observed no city of chicago certified food sanitation manager on site nor city of chicago food service sanitation certificate while serving and handling tcs foods (chicken, taquitos, pizza, etc. Instructed must have a certified food sanitation manager on site at all times when tcs foods are being handling and/or served. Priority foundation violation 7-38-012. Citation issued
16. Food-contact surfaces: cleaned & sanitized
- comments: food ice not protected from contamination during storage. Observed black and brown slime substance in the interior contact surfaces of ice machine. Slime likely to contaminate ice for human consumption. Instructed to discontinue using machine until service cleaned and sanitized. Priority foundation violation 7-38-005. Citation issued.
38. Insects, rodents, & animals not present
- comments: observed approximately 20 live fruit flies on floor drains behind refrigeration units and on walls in rear food storage area. Also observed 8 live house flies in front display area and rear food storage areas. Must clean and sanitized affected areas. Additional pest control service is needed to eliminate flying insect problem. Priority foundation violation. 7-38-020(a) citation issued.

- comments: observed excess food debris on floor and floor drains behind all refrigeration units, coffee station area, inside walk-in cooler and floor throughout rear storage area. Instructed to detail clean floor throughout premises. Must maintain floor dry at all times to prevent pest breeding.

- comments: observed clutter cardboard boxes and unnecessary items in rear food storage area. Instructed to remove clutter and unnecesary items to prevent rodent harborage. Instructed to elevate items 6inches/wall in rear storage area. Instrcuted to use approved shelving for product elevation.

- comments: observed a few water damaged ceiling tiles in rear of premises. Musr repair/replace and maintain.

- comments: observed dust build up on ventilation vents throughout premises. Must clean and maintain.
