- comments: observed no city of chicago certified food manager on the premises while tcs foods (lamb, chicken) were being prepared and served. Instructed management: a city of chicago certified food manager must be present at all times while tcs foods are being prepared or served. Priority foundation 7-38-012. Citation issued.

16. Food-contact surfaces: cleaned & sanitized
- comments: found low temperature dish machine not sanitizing; chlorine reading 0 ppm. Priority violation 7-38-025. Citation issued. Dish machine has been tagged and may not be used until reinspected by cdph and sanitizing according to data plate.

38. Insects, rodents, & animals not present
- comments: observed approximately 50 rat droppings on the floor near the grease trap and adjacent storage room located in the basement. Management instructed to clean and remove all rodent droppings from all affected areas and contact pest control to eliminate all pest activity from the premises. Priority foundation 7-38-020(a). Citation issued.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed facility without a chemical test kit on site for the sanitizer (chlorine) being used for the low-temperature dish machine or the sanitizer (q u a t) being used at the 3-compartment sink. Instructed management that a test kit that accurately measures the concentration in mg/l of a sanitizing solution must be provided. Priority foundation violation 7-38-005. Citation issued.

- comments: found heavy ice and food debris build up in the three chest freezers located in the basement. Must defrost, remove ice, clean, and maintain.

- comments: observed a leak at the prep/dish area handwashing sink faucet. Must repair.

- comments: found tile flooring without grout in the cooks line area. Must regrout tile flooring and maintain.

- comments: must replace burned-out light bulbs inside the ventilation hood to provide adequate lighting and maintain.

- comments: observed previous core violations from 3/22/22 report #2553252 not corrected: violation #10 (6-301. 14) observed no hand washing signage at hand sink at bar. Must provide. Violation #37 (3-302. 12) observed no label on food containers in the basement (spices). Instructed to provide common name to identify properly. Violation #47 (4-101. 19) must remove foil used as liners on shelves throughout the kitchen prep area. Violation #47 (4-102. 11) observed grocery bags used to store frozen chicken in the chest freezer. Instructed to use food-grade containers to store items. Violation #53 (5-501. 17) women's bathroom must have a trash can with a lid covered. Violation #55 (6-501. 114) observed excessive clutter in basement storage with wooden doors and other unnecessary items. Must remove from premises. Must correct previous core violations. Priority foundation 7-42-090. Citation issued.
