3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed several employees who have not signed the employee health policy. Management instructed to have a signed employee health policy from all employees. Priority foundation 7-38-010.

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean up supplies on the premises to respond to vomit and diarrheal events. Management instructed to provide. Priority foundation 7-38-005.

9. No bare hand contact with rte food or a pre-approved alternative procedure properly allowed
- comments: observed a food handling employee handle ready-to-eat food (pita bread) with their bare hands while bagging the bread after it came off of the conveyor belt. Management instructed that food handling employees cannot touch ready-to-eat foods with their bare hands. Priority 7-38-010. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the exposed hand washing sink in the kitchen prep area not accessible for use during the inspection. There were metal mixer parts and a plastic container stored inside of the exposed hand washing sink in the kitchen prep area. Management instructed to maintain all hand washing sinks accessible for use at all times. Priority foundation 7-38-030(c). Citation issued.

- comments: no hand soap was provided at the exposed hand washing sink in the kitchen prep area. Management instructed to provide hand soap at all hand washing sinks at all times. Priority foundation 7-38-030(c). See violation #10 above for citation.

38. Insects, rodents, & animals not present
- comments: observed evidence of rodent activity on the premises and live insects in the food preparation area. Observed 5 mouse droppings on the floor of the kitchen prep area between the 1 burner stoves and oven. Management instructed to clean and remove all rodent droppings from the premises. Also observed 4 live cockroaches on the floor and walls of the kitchen prep area. 10 small flying insects were observed in the connected grocery store located at 4738 n. Kedzie. Additional pest control service is needed to eliminate the pest activity throughout the premises. Priority foundation 7-38-020(a). Citation issued.

38. Insects, rodents, & animals not present
- comments: observed dead cockroaches in the kitchen prep area on the exposed hand washing sink, the prep table and on dirty dishes stored on the drainboard of the 3 compartment sink. Management instructed to clean and remove all dead insects.

40. Personal cleanliness
- comments: observed several food handling employees in the bakery prep area not wearing a hair restraint while working with food. Management instructed that all food handling employees must wear a hair restraint in the food prep areas.

49. Non-food/food contact surfaces clean
- comments: detail clean the food prep tables, interior of oven, and the hand washing sinks in the food prep areas.

55. Physical facilities installed, maintained & clean
- comments: clean the walls and ceiling in the bakery production area. Clean the floor in the walk-in freezer.

56. Adequate ventilation & lighting; designated areas used
- comments: must provide light shields for unprotected light fixtures in the bakery production area.

57. All food employees have food handler training
- comments: observed several food handling employees without a food handler certificate. Must show of training for all employees.

60. Previous core violation corrected
- comments: previous core violations from report #2464101 on 12/28/20 not corrected: 55 - instructed to provide smooth, easily cleanable ceilings over food preparation areas and maintain. Priority foundation 7-42-090.

64. Public health nuisance
- comments: observed 3 employees not wearing a face covering inside of the bakery. Management instructed that all employees must wear a face covering at all times. Citation issued 7-28-060.
