- comments: person in charge is not a city of chicago certified food manager. Priority foundation violation, 7-38-012.
2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food manager on the premises while tcs foods (chicken, beef, rice, noodles, etc. ) were being prepared and served. Instructed management that a city of chicago certified food manager must be present at all times while tcs foods are being prepared or served. Priority foundation violation 7-38-012, citation issued. Food manager arrived on site after inspection at 2:43 pm. -cos.
15. Food separated and protected
- comments: observed improper food storage in the walk-in cooler. Observed containers of raw chicken and raw pork stored over bean sprouts and cooked chicken. Instructed management that food shall be protected from cross contamination by arranging all foods in order of minimum cooking temperatures with raw foods stored at the bottom and cooked foods stored above. Facility rearranged food items correctly at the time of inspection. -cos. Priority violation, 7-38-005. Citation issued.

- comments: observed torn gaskets on the inside of sandwich prep cooler. Instructed to repair and maintain.

- comments: observed raw meats stored in single service grocery bags in the freezer in the basement. Instructed management not to store raw meats in single use bags and to store food items in food grade containers or bags only.

- comments: observed a leak underneath handwashing sink in the smoothie station. Instructed to repair and maintain.
