- comments: observed no person in charge on site during inspection. Instructed manager to designate person in charge during all hours of operation. Priority foundation

- comments: observed no certified manager on site or valid certificate posted when tcs food (pizza 135. 7f) prepared and served. A certified manager must be on site at all times tcs food prepared and served. Priority foundation 7-38-012 citation issued

- comments: observed no employee health policy available. Instructed manager to provide. Priority foundation 7-38-010
5. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean up procedures on premises for vomiting and diarrheal events. Instructed to provide. Must provide all the necessary items in policy including disposable gloves, face mask, disposable mop with appropriate sanitizer for the norovirus. Priority foundation violation 7-38-005.

- comments: observed no hand washing sink in front pizza serving area. Instructed manager to install hand washing sink with hot and cold running water under city pressure, soap and paper towels in front pizza serving area. Priority foundation 7-38-030(c)

- comments: observed deep dish pizza displayed for service and stored at improper temperature 120. 9f in front pizza sering area. Instructed manager to discard. All hot food must maintain 135f or above during storage, display and service. Priority 7-38-005 refer to violation #22 for citation

- comments: observed the following cold tcs foods stored at improper temperature in prep cooler, raw ground beef 49. 4f, raw hamburger patties 53. 2f, raw sausage 53. 2f. Instructed manager to discard approximately 20lbs and $50 of product. All cold tcs food must maintain 41f or below during storage, preparaion, display and service. Priority 7-38-005 citation issued

- comments: observed pizza prep cooler not maintaining proper temperature 53. 2f cooler is tagged held for inspection and must not be used until reinspected by chicago department of health. Priority 7-38-005 citation issued

- comments: observed 1/4' gap on bottom of rear door. Instructed manager to seal all outer openings on door to prevent pest entry.

- comments: observed food items stored or floor in walk-in freezer. Instructed manager to elevate all items six inches off floor.

- comments: observed middle door not properly closing and in poor repair on pizza prep cooler. Instructed manager to repair and maintain

- comments: observed accumulated food debris and grease buildup on interior and exterior surfaces of cooking equipment, prep tables, coolers, and freezers. Instructed manager to clean and maintain.

- comments: observed drainpipe leaking under hand washing sink in washroom and hand washing sink slowly draining in dishwashing area. Instructed manager to repair and maintain.

- comments: observed no covered waste receptacle in washroom. Must provide.

- comments: observed damaged and missing baseboards along walls in prep, dishwashing and storage areas. Must replace and maintain.
