- comments: observed establishment with outdated lab results for pork grasitas report of analysis from 1/12/2009 provided on premises. Instructed manager must provide and maintain an updated lab letter on mentioned foods on premises. Lab result analysis must be done every 5 years as required. Priority foundation violation 7-38-005, citation issued.
38. Insects, rodents, & animals not present
- comments: observed opening gap at left corner of rear west side exit door. Must pest proof mentioned door to prevent pest entry and maintain.

- comments: instructed not to use grocery bags for food storage inside reach-in-cooler at front taqueria prep area. Must use food grade storage bags or containers.

- comments: observed employee unisex washroom not self closing located in basement dry storage areas. Must install self closing device on mentioned washroom door and maintain.

- comments: observed unused vendor refrigeration units stored in basement dry storage areas. Must remove from the premises due to potential pest harborage.

- comments: observed expired food handler certificates provided on the premises. Must provide and maintain current proof of training for all food handling employees.
