Report date: Sep 19, 202522. Proper cold holding temperatures
- comments: observed cold tcs foods, held at improper temperatures ranging between 46. 9-50. 4f. Observed cold tcs foods, milk(50. 4f), mozzarella(48. 0f),blue cheese crumbles(49. 8f), sliced tomatoes(46. 9-47. 7f) and mixed green salad(50. 4f)held at improper temperature. Instructed all cold tcs food must be held at 41f or below. Corrected on site manager voluntarily discarded and denatured food valued at $60 weighing 17lbs. Priority violation 7-38-005 citation issued.
33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed two door(lowboy 9 right) cold holding unit in prep area on 9th floor at improper ambient temperature of 46. 9f. Instructed all cold holding units must be maintained at 41f or below. Held for inspection tag placed on two door(lowboy 9 right) cold holding unit in prep area on 9th floor. Once two door(lowboy 9 right) cold holding unit in prep area on 9th floor has been repaired and able to maintain tcs foods at 41f or less e-mail food@cityofchicago. Org to request tag removal prior to using unit. Priority violation 7-38-005 citation issued
39. Contamination prevented during food preparation, storage & display
- comments: observed no ice bin covers/lids for ice bins in bar 1, bar 5 (left bar) and bar 6. Instructed to provide covers/lids for ice bins in said bars to prevent contamination of ice in ice bins during storage to prevent exposure to splashes, dust or other contamination. Observed no splash guard on handwashing sinks in bars(bar 5, bar 6 ) and at handwashing sink in 9th floor prep area next to ice bin. Instructed to install left splash guard on handwashing sink in right bar and right splash guard in left bar in bar 5, install right splash guard on handwashing sink in bar 6 and install left splash guard on handwashing sink next to ice bin in prep area on 9th floor in kitchen to prevent contamination of ice in ice bins.
51. Plumbing installed; proper backflow devices
- comments: observed no prep sink in bars in facility. Observed no prep sinks installed in left and right bar in bar 5 and bar 6. Instructed to install a minimum of one prep sink for food preparation, obtaining water and discarding liquid waste is required in any food preparation area.

- comments: unable to verify existence of backflow prevention devices on coffee machines in bar 5 and in 9th floor prep area. Instructed to install backflow prevention devices in said areas so that it shall be located so that it may be serviced and maintained.
