- comments: no hand soap is located at the kitchen handwashing sink. Must provide at all times. Priority foundation violation 7-38-030(c) citation issued.

- comments: no paper towels are located at both the front and rear prep handwashing sinks. Must provide at all times. See the above violation #10 for citation.

- comments: interior ice machine with black slime substance. Must clean.
23. Proper date marking and disposition
- comments: noted prepared tcs foods such as cooked egg rolls, cooked chicken, cooked stuffed dumplings throughout coolers with no date labeling. No expiration dates of prepared foods, no dates of any kind. All foods discarded. Instructed all prepared tcs foods held longer than 24 hours must have an expiration date no longer than 7 days. Priority foundation violation 7-38-005 citation issue.

- comments: all bulk food containers must be labeled with the product name.

- comments: observed hundreds of mice droppings throughout the facility. Droppings on the cooks line shelving, along baseboards under cooking equipment, under the cooks line coolers, under all dry storage shelving in front and rear, floors under the dishwashing. Throughout the bulk food storage room on floors and bulk food containers. On floors in the washroom. Throughout the rear storage room. Instructed to have pest control co. Service for mice, clean and sanitize all areas, remove all droppings. Priority violation 7-38-020(a). Citation issued.

- comments: all wiping cloths in all prep areas must be held in a container with a sanitizing solution.

- comments: noted knives stored between the prep and steam tables. Must provide proper knife holder.

- comments: all stored items throughout the front prep and rear storage must be elevated from the floors.

- comments: must not use plastic grocery bags as food containers in all coolers and freezers. Containers must be washable or food grade plastic bags. ----rear storage box freezer lids broken and in poor repair and repaired with duct tape. Must replace.

- comments: all shelving throughout entire establish with dirt, grease and food debris. Must clean. ----all raw wooden shelving inside the box freezer must be painted/sealed.

Must clean. ----water damaged ceiling tiles throughout the facility must replace. ----excessive dust in the ceiling ventilation above the cooks line. Must clean.

- comments: previous core violation from 6-30-21 report #2522122 not corrected. #49- 4-602. 13- found following not clean(excessive grease): wok areas and deep fryer areas. #55- 6-501. 114-- found clutter at rear storage areas. (old boxes,equipment,etc. ) must clean and organize rear storage areas. Priority foundation violation 7-42-090 citation issued.
