- comments: person in charge at the time of inspection does not have a city of chicago sanitation certificate. Priority foundation violation. No citation issued -
2. City of chicago food service sanitation certificate
- comments: no city of chicago certified food manager was on-site during the inspection when foods were being prepared and served(hamburger,rice,etc). The manager stated via phone 'i'm at home'. Also no valid city sanitation certificate posted on site. Reviewed that a city of chicago certified food service manager must be on-site at all times when foods are prepared and served. Instructed to obtain a valid city sanitation certificate,information given. Priority foundation: 7-38-012, citation issued. On 1-5-2022 returned on site to write report and citation,again no city of chicago certified food manager on-site
10. Adequate handwashing sinks properly supplied and accessible
- comments: no paper towels provided at handwashing sink at front prep/serving area. A washcloth hanging at splash guard of the sink which will be used for all-day the same cloth. Instructed no cloth to be used unless is a retractable cloth unit which continuously provides a clean cloth. Upon my request employee provided a roll of paper towel. Priority foundation violation:7-38-030(c), citation issued.

- comments: label dry food containers with common name at rear storage area(rice, etc)

- comments: observed cooks knives being stored improperly on top dirty wall ledge under electrical boxes in prep/dishroom. Instructed to wash, rinse and sanitize mentioned items and must provide a proper knife holder. Ice scoop must be stored in a clean container between use
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: rusted basin of prep sink. Base of hot holding unit extremely corroded, unable to clean. Must repair and restore to a smooth, easily cleanable surface or replace mentioned units. Soda dispenser at rear prep area, valves attached at soda boxes are broken, leaking on other boxes and floor, excess syrup on boxes, equipment, and floor. Must replace. The metal front plate at the interior of the ice machine is rusty and broken. Instructed to replace, surface must be smooth cleanable and non-absorbent material
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed employee washing, rinsing, and store pots and plastic basket used by customers at three-compartment sink which was not set-up. No sanitizing solution was used at the third compartment sink. Personally set up three-compartment sink:wash,rinse and sanitize(chlorine 100ppm). Priority foundation:7-38-005,citation issued

- comments: accumulated grease and food debris on walls, utility connections, and equipment surfaces throughout, also interior and exterior of cooking equipment. Must clean and maintain.

- comments: previous core violations not corrected from 3-29-21; report# 2497531 #51) 5-205. 15 observed hot water handle of handwashing sink outside of walk-in cooler in disrepair. Repair and maintain. #55) 6-201. 11 observed floors unclean throughout with an accumulation of dirt and grime. Must clean all floors in corners and along walls.
