- comments: observed over 30 dead insects and flies in light shields in the lobby area and approx 8-10 dead insects in the light shields in the food prep area. Instructed to clean and maintain all.

- comments: found a heavy accumulation of grease, dirt, and debris on the floors throughout the facility (inside the walk-in cooler, underneath storage shelving and cooking equipment, in the front register area, in the dishwashing area). Must detail clean all floors and maintain.
