2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food manager on site during time of inspection while preparing and serving tcs foods (deli meat sandwiches, italian beef, nacho cheese, corn). Instructed to provide at least one certificate holder on site while open and operating. Priority foundation violation. #7-38-012. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand cleanser available at handsink located in prep area. Instructed to provide and maintain for proper hand washing. Priority foundation violation. #7-38-030 (c). Citation issued. Pic provided at this time.

- comments: observed no hand drying provision available at handsink located in prep area or handsink located in rear dishwashing area. - comments: missing thermometers inside walk in cooler. Instructed to provide and maintain.

38. Insects, rodents, & animals not present
- comments: observed approximaely 20 small and large live black flies flyring throughout prep, cashier area, storage and walk in cooler. Instructed to contact pest control for additional service, must eliminate fly activity. Priority foundation violation. #7-38-020 (a). Citation issued

40. Personal cleanliness
- comments: observed food handlers preparing food without hair restraints. Instructed to provide.

49. Non-food/food contact surfaces clean
- comments: observed all cooling units located in prep area in need of cleaning on interior and exterior surfaces. Must remvoe dried food debris.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed refuse area with excessive amounts of clutter, boxes, crates, miscellaneus items all throughout ground area. Instructed to remove in order to prevent hiding spaces for pest and maintain refuse area.

55. Physical facilities installed, maintained & clean
- comments: observed items stored on floors walk in cooler, rear prep and storage areas. Instructed to provide adequate shelving units and remove all dried food debris from floors including grocery area.

57. All food employees have food handler training
- comments: observed food handlers without training certifications. Instructed to provide and maintain on site for review.

58. Allergen training as required
- comments: instructed to provide alleren training for city of chicago certificate holder and maintain on site.
