Report date: Apr 23, 202110. Adequate handwashing sinks properly supplied and accessible
- comments: observed small room in basement being used as a prep area with no access to handwashing. Observed plugged in meat slicer with food debris on meat slicer. Observed knife rack on wall and prep table. All items need to be removed from room or handwashing sink with hot and cold running water needs to be installed. Priority foundation violation 7-38-030 (c) citation issued.

- comments: observed can opener unclean. Must clean and maintain.
22. Proper cold holding temperatures
- comments: observed tcs food at improper temperatures within cooler including cooked potatoes at 57. 2 f, sliced turkey and ham deli meat at 47. 5f, corned beef hash at 47. 1f. All food was discarded. Priority violation 7-38-005
33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed cooler containing tcs foods including cooked potatoes, sliced deli meat, and corned beef hash at 45f. Cooler was tagged during inspection. Priority violation 7-38-005 citation issued
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed chest freezer in dishwashing area with top with peeling paint and rust. Surface needs to be restored to a smooth easily cleanable condition and maintained.
55. Physical facilities installed, maintained & clean
- comments: observed foam sealant used to seal around basement door in kitchen. Foam sealant is not smooth and easily cleanable. Must be removed and door must be sealed properly. Observed floor under cooler in kitchen with holes you can see into basement. Floor must be repaired and restored to a smooth easily cleanable condition.

- comments: observed previous core violations from report dated 3/23/2020 not corrected including #55 in basement two rooms are filled with unecessary and construction items. Instructed to remove what is not needed and elevate 6 inches from floor what is needed. #55 broken wood ceiling not smooth and easily cleanable. Debris from ceiling on food items. Instructed to install ceiling throughout basement area. Priority foundation 7-42-090
