- comments: noted no soap available at the three-hand washing sinks located at the kiosks of "savory stack, " "salad bar," and "deli. " hand sanitizer only was provided in the soap dispensers. Management provides soap. Priority foundation violation 7-38-030(c) citation issued.
22. Proper cold holding temperatures
- comments: noted approximately 10 lbs of cooked chicken chunks in a 3-inch deep metal container of the upper portion service cooler at "savory stacks" with an improper cold holding temperature of 91. 6f. Also noted were cooked mushrooms in the same cooler at 44. 6f, both with today as a prep date. Reviewed proper cooling methods of cooked foods and proper cold-holding temperatures of tcs foods. All foods are discarded. Priority violation 7-38-005 citation issued.
38. Insects, rodents, & animals not present
- comments: observed approximately 50 live small drain flies at the dish machine area around the food waste disposal machine and the conveyor belt for soiled dishes. Recommend having pest control co. Service area for flies, eliminate all moist, organic material, and eliminate fly activity. Priority foundation violation 7-38-020(a) citation issued.
39. Contamination prevented during food preparation, storage & display
- comments: must provide a splash guard on the right side of the south wall handwashing sink in the spice food prep area.

- comments: torn rubber door gaskets on the tall reach-in cooler at the "paseo" food station. Must replace.

- comments: noted food-soiled and stained wooden cutting boards of metal food cart at the rear kitchen cooks line. Must clean or replace.

- comments: the interior of ovens with excessive food debris must clean and maintain. Table top can opener with encrusted food debris. Must clean after each use.

- comments: dirty floor drain under the dish area and soiled dish conveyor belt. Must clean and maintain.

- comments: no proof of allergen training or certificates for all city of chicago certified food managers. Must provide.
60. Previous core violation corrected
- comments: previous core violations from report #2550114 on 2-18-22 were not corrected. #44- observed a prop/display wooden bowl not acceptable for food storage being stored on shelving with clean food containers. Must not store non-food contact equipment with utensils/food containers used for food storage. #51-must remove all eyewash stations from all hand washing sinks to prevent possible cross contamination. #55-excessive pooling water throughout the floors under and around the dish machine. Must remove and maintain dry floors. Priority foundation violation 7-42-090 citation issued.
