1. Person in charge present, demonstrates knowledge, and performs duties
- comments: no person in charge on site at time of inspection. Must have a person in charge on site at all times. Priority foundation violation. 7-38-012. Person in charge arrived on site during inspection.

2. City of chicago food service sanitation certificate
- comments: no certified food manager on site while tcs foods were being prepared on site(pork, sausage). Instructed to have a cfm on site at all times while tcs foods are being prepared and served. Priority foundation violation. 7-38-012. Citation issued. Cfm arrived on site approximately 2:20 pm.

8. Hands clean & properly washed
- comments: observed employees not washing hands before putting on gloves and went directly to preparing food (pork, sausage). Instructed to wash hands immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single- service and single-use articles priority violation. 7-38-010. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand washing sink located in main prep area. Instructed to install hand washing sink with hot and cold running water connected to city of chicago plumbing line. Hand sink must also be equipped with soap and paper towels. Priority foundation violation. 7-38-030(c). Citaiton issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: no paper towels at hand washing sink located at juice prep area. Must provide paper towels at hand sink at all times. Priority foundation violation. 7-38-030(c). See above for citation. Employee replenished paper towels at hand sink located at juice station.

13. Food in good condition, safe, & unadulterated
- comments: observed pork unsafe to eat for human consumption. 3 containers of pork had mold like white, black and green spots on the food stored in prep cooler. Food is voluntarily discarded. Food weighing approximately 10 lbs at a cost of $50. 00. Priority violation. 7-38-005. Citation issued.

22. Proper cold holding temperatures
- comments: observed shelled white eggs at improper cold holding temperature at 68f. Shelled eggs were sitting out on the counter at room temperature. Tcs foods must be held at 41f or below inside of a cold holding unit. Priority violation. 7-38-005. Citation issued. Food discarded. Food weighing approximately 5 lbs at a cost of $12. 00.

39. Contamination prevented during food preparation, storage & display
- comments: observed opened boxes of food stored on the floor near rear door such as whole lettuce head, plaintains and corn husks. Instructed to store food in a clean dry location and at least 6 inches off the floor.

52. Sewage & waste water properly disposed
- comments: observed sewage backup at 3-compartment sink. Instructed to repair plumbing issue. Priority violation. 7-38-030(c). Citation issued. Plumber arrived on site and corrected plumbing issue. Water is draining into approved drain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed toilet rooms have standing water on the floors. Must clean and maintain toilet rooms.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed numerous bags of trash throughout the establishment. Instructed to remove trash from the premises and store inside outside dumpster receptacle.
