- comments: observed 1/4'-1/2' gap surrounding in unit fan on wall in ware washing area. Instrcuted to make tight fitting and maintain.

- comments: observed mice activity in rear storage areas, under cashier counters, under chips and soda aisles. Approx 100 droppings. Observed approximately 15 live roaches in rear prep area near washroom, water heater and furnce room. Instructed manager to clean all affected areas and contact pest control for a complete service. Priority foundation 7-38-020 (a) citation issued.

- comments: observed no splash guard between prep sink and 3-compartment sink, and between hand sink and cutting board in meat prep area. Instructed manager to provide and maintain.
