- comments: observed consumer advisory not correct on menu. Advisory is missing disclosure statement ('items can be served raw or undercooked or contain undercooked ingrediants'). Instructed management to add disclosure statement to current advisory and indentify raw/undercooked foods by asterisking them to the advisory. Priority foundation 7-38-005. �citation issued.

- comments: observed bulk food container on premises without a label (sugar). Instructed to provide the common name on bulk containers to properly identify food item.

- comments: found heavy ice build up inside the chest freezer near the stairs in the basement. Must defrost, remove ice, clean, and maintain.

- comments: found the mens restroom door not self-closing. Must repair so that door closes.

- comments: found the floor tiles behind the bar in poor repair; tile flooring is broken and exposing unsealed wood that is collecting food debris and dirt. Found the ceiling above the ice machine in poor repair; the ceiling panel is broken and exposing insulation. Must repair said floor and ceilng.

- comments: observed clutter and unnecessary items in the rear basement area. Instructed management to remove clutter/unnecessary items, and clean area to prevent rodent harborage.

- comments: found heavy dust accumulation on the fan in the walk-in cooler. Must clean and maintain.

- comments: previous core violation from 6/4/2021 report #2510810 not corrected: violation #47 (4-101. 19) observed rusty storage racks inside walk-in cooler and rear storages rooms. Must remove rust or replace shelving. . Must correct previous core violations. Priority foundation 7-42-090. Citation issued.
