- comments: observed food not in original containers must label and maintain.

- comments: observed evidence of appx. 60 or more mice droppings on floor under shelving units in produce walk in cooler and along wall bases behind produce storage in rear area. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 4/21/21. (priority foundation 7-38-020(a)(citation issued)

- comments: observed an appx. '1-2' gap in center of front entry & exit doors and rear receiving dock door and floor plate. Must make tight fitting.

- comments: must provide a splash guard at hand sink next to prep table in fish dept.
