- comments: observed 1/4' gap on bottom right corner of exit door in mechanical room. Instructed manager to seal all outer openings on door to prevent pest entry.

- comments: observed over 30 mice droppings scattered on floor along walls and corners in dry storage area, 30 mice droppings scattered on floor along walls and corners inside furnace room, 30 mice droppings scattered on floor along walls and corners in mechanical room, 20 mice droppings scattered on floor in corner behind door in beverage/dishwashing area, 20 mice droppings scattered on floor along walls and corners behind cooking equipment, 20 mice droppings scattered on floor along walls behind beverage equipment, 10 mice droppings scattered on bottom of prep table inside muffin pans. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued.

- comments: observed food items stored on floor in prep and storage areas. Instructed manager to elevate all items six inches off floor.

- comments: observed multi-use utensils and equipment not inverted. Instructed manager to invert utensils and equipment to protect from contamination.

- comments: observed single service spoons not inverted. All single service articles must be inverted to protect from contamination.

- comments: observed milk crates used as shelving units in storage areas. Instructed manager to provide adequate shelving for easy cleaning and pest control.

- comments: observed accumulated grease buildup and food debris on interior and exterior surfaces of cooking equipment, prep tables, shelving units. Instructed manager to clean and maintain.

- comments: observed no covered waste receptacle in unisex washroom. Instructed manager to provide.

- comments: observed dust, dirt and debris on floor along walls and corners in prep, storage, furnace room, utility, and mechanical areas, debris inside floor drain near prep sink, dust buildup on ceiling vents and light shields in prep and dish washing areas. Instructed manager to clean and maintain.

- comments: observed missing baseboards near rear door, damaged wall surfaces in prep and storage areas. Instructed manager to repair and maintain.
