- comments: pic doesn't have a food managers certificate. Must provide and maintain. (priroity foundation 7-38-012)( no citation issued)

2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty while tcs foods are being prepared,handled and served such as (chicken,coleslaw,french fries,etc) must be on site at all times. (priority foundation 7-38-012)(citation issued)

- comments: must label all food storage containers when food is not in original package.

38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 30 or more live small flies flying around in toilet room with soiled wet filters in mop sink,dish washing area under sink with a container of water leaking from pipe,prep area has food build up on walls. Must remove items that area conducive and maintain. Recommended to have a pest control operator to service premises on or after 2/5/21. (priority foundation 7-38-020(a)(citation issued)

- comments: observed an appx. '1/2' gap along bottom of rear door. Must make door tight fitting.

- comments: must clean grease and food debris build up from prep tables,fryer cabinets,pipes of cooking equipment,wheel bases,personal fans. Must defrost chest freezer.

- comments: observed outside garbage dumpsters overflowing with trash above rim and trash bags on ground around dumpster. Must place trash inside of dumpsters and lids to be tight fitting. (priority foundation 7-38-020(b)(citation issued)
