- comments: observed no city of chicago certified food manager on the premises nor city of chicago food manager certificate posted while tcs foods (chicken wings, pizza) were being prepared and served. Instructed management that a city of chicago certified food manager must be present at all times while tcs foods are being prepared or served. Priority foundation 7-38-012. Citation issued.

- comments: found no pest control log book on site at time of inspection. Instructed management to provide a pest control log book with up to date service reports and all other necessary documents that are needed inside log book such as pest control license, chemicals used, map, etc. And to keep log book on site. Priority foundation 7-38-020(c). Citation issued.
55. Physical facilities installed, maintained & clean
- comments: found an accumulation of food and dirt on the floor underneath the oven in the front prep area. Instructed management to said area and maintain.
56. Adequate ventilation & lighting; designated areas used
- comments: found heavy dust and grease accumulation on the ventilation hood located over the oven and heavy dust build up on the ventilation cover inside the restroom. Instructed management to clean said areas and maintain.
