- comments: observed no certified manager on site at time of inspection while tcs items such as hamburgers were being prepared. Instructed to maintain an employee holding a valid city of chicago foodservice manager certificate on site at all times while open and operating. Priority foundation violation 7-38-012. Citation issued.

22. Proper cold holding temperatures
- comments: observed the following tcs items at the stated improper temperatures inside 2 prep coolers located in main prep area: tzatziki sauce at 44. 7f, guacamole at 47. 1f, cooked pork at 46. 7f, and cheese at 56. 1f. Management voluntarily discarded and denatured approximately 15lbs of product valued at approximately $25. Instructed to maintain all tcs items at or below 41f at all times. Priority violation 7-38-005. Citation issued.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed 2 coolers in main prep area containing tcs items such as cooked pork and guacamole maintaining improper ambient temperatures of of 48. 2f and 48. 1f. Instructed to maintain coolers containing tcs items at or below 41f at all times. Priority violation 7-38-005. Citation issued. See comments.

- comments: observed torn gasket on prep cooler opposite flat top grill. Instructed to repair or replace and maintain in good condition.

- comments: observed no covered trash receptacle inside all gender restroom on east side of building. Instructed to provide.

55. Physical facilities installed, maintained & clean
- comments: observed build up of food debris on slicer in basement and wall behind low temperature dish machine. Instructed to detail clean and maintain in good condition.
