- comments: observed no certified food managers certificate posted to view while tcs foods are bein prepared,handled and served such as (italian beef,chicken wings,etc). Must provide and post. (priority foundation 7-38-012)(citation issued)

- comments: observed no clean up kit for vomiting and diarrhea. Must provide and maintain. (priority foundation 7-38-005)(citation issued)

- comments: observed evidence of appx. 125 or more mice droppings on floor on both sides & at ledges of unused cooler,on floor in office along wall base,along wall bases in prep area,on lower shelf of unused steam table,on floor in hot water tank area,under 3- compartment sink,under all dish storage shelves,inside of boxes of menus,ledge of dolli. Also. Evidence of appx. 8 or more live small flies around 3- compartment sink. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 10/1/20. (priority foundation 7-38-020(a)(citation issued)
