- comments: observed no hand washing sign at hand sink in rear prep area. Must provide and maintain.

- comments: observed improper temperatures of tcs foods such as. 5lbs. Cut lettuce 71. 8f on counter top. 1lb. Pre-cooked pizza puffs 66. 4f. 6 oz. Pre-cooked fish patties 70. 2f. 1lb corn dogs 67. 5f. 1lb. Breaded chicken 57. 6f inside of 1- door prep cooler. Product was discarded by manager. Must have cold holding at 41. 0f or below. - comments: observed evidence of appx. 30 or more live small flies on a soiled paper towel on floor behind ice machine,on walls and flying around in prep areas and dining area. Recommended to have a pest control operator to service premises on or after 7/7/2021. (priority foundation 7-38-020(a)(citation issued)

- comments: observed an appx. '1/2-3/4' gap along bottom of entry and exit doors. Must make doors tight fitting.

- comments: must clean excessive grease and food debris build up on interior and exterior of fryer cabinets,prep tables,grill table,walk in cooler- shelving units.

- comments: must repair or replace leaky faucet and leaky gasket underneath far right compartment at 3- comp sink.

- comments: must repair or replace damaged & loose wall bases in dining area & dish washing areas,stained ceiling tiles in dining area,damaged and broken concrete throughout rear storage area and mop sink/ice machine area.
