- comments: observed 56 mice droppings in lower storage area on floor and storage shelves, live roaches, and small flies. Instructed to clean/ sanitize all areas. Recommened pest service. Priority foundation issued 7-38-020(a).

39. Contamination prevented during food preparation, storage & display
- comments: observed sliced tomatoes,lettuce, etc stored in grocery bags. Instructed to use only food grade storage bags.

55. Physical facilities installed, maintained & clean
- comments: must properly store mop after use and clean mop bucket to prevent insect breeding.
