- comments: observed no employee health policy available. Instructed manager to provide signed acknowledgement of employee responsibility to report any illness transimssible through food. Priority foundation 7-38-010 citation issued
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand washing signs at hand washing sinks in prep and washroom areas. Instructed manager to provide hand washing signs at all hand washing sinks used by employees.
38. Insects, rodents, & animals not present
- comments: observed over 200 mice droppings scattered on floor under shelving units and along walls near rear walk-in cooler and freezer, along walls behind cooking equipment and ice machine, inside compressor units and bases of prep tables and holding units, on floor along walls near walk-in cooler in pizza prep area and beverage cooler in front counter area, on floor along walls and in all corners in office, prep, dry storage, laundry, electrical, utility, washroom and rear storage areas. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed no pest contol log book. Instructed manager to provide. Premises must be serviced by a licensed exterminator and log book provided upon inspection. Priority foundation 7-38-020(c) citation issued

- comments: observed food items stored on floor in walk-in freezer. Instructed manager to elevate all food items six inches off floor.

- comments: observed accumulated food spills, debris and grease buildup on interior and exterior surfaces of cooking equipment, prep tables, holding units, shelving units in prep and storage areas. Instructed manager to clean and maintain.

- comments: observed missing outlet covers, openings in walls and ceiling, along baseboards, etc in prep and storage areas. Instructed manager to seal all openings in walls and ceilings and maintain.
