- comments: observed evidence of and infestation of mice droppings of appx. 300 or more scattered throughout premises such as at drive thru- service counter shelving units and window ledge,on floor under front service counter along wall base and behind safe,on lower shelving units of hot holding unit ,on lower shelves of cup & lids storage front counter,on lower shelf under cold chicken prep line behind pans,on biscuit prep table,inside a box of sandwich bags and prep table as well,on floor in walk in cooler along wall bases and under shelving units,inside and on top of boxes in rear dry food and paper storage,under shelving units in same area,on floor along wall bases in electrical room and on counter top underneath lobby soda machine. Must remove droppings. Clean and sanitize areas. Must have a pest control operator to service premises on or after 3/22/22. (priority 7-38-020(a)

- comments: must repair or replace worn door gasket on rear hot holding unit,lower shelf damaged in walk in freezer.

- comments: observed accumlated food debris and grease buildup on interior and exterior surfaces of fryers, coolers, prep tables, holding units, shelving units, bulk containers, walk in cooler shelving units. Instructed manager to clean and maintain.

- comments: observed no backflow device on ice machine to be seen for servicing. Must provide and maintain.
