High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 1 dead fly on liner with to go cups in use and 1 dead fly in hand washing sink. Observed 7 live flies on wall by prep table where meat slicer is stored in preparation area. Observed 9 live flies on lids above prep table used to cover bus pans. Observed 8 live flies on wall in front of preparation table in front of meat grinder. Observed 8 flies on walls on shelf above prep line above prep area where chicken is being prepared in meat grinder. Observed approximately 20 live flies on plantains. Observed 5 flies flying around, landing on walls and brown to-go bags, and aluminum singe service to go containers located in dry storage area separated from kitchen.
High Priority - Raw animal food stored over ready-to-eat food. 1)Observed raw chicken stored above raisins in walk in cooler 2) Observed raw beef stored above open container with potatoes in walk in cooler. Observed raw pork being held above cilantro in walk in cooler Operator rotated all products.
High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse.
High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit.
High Priority - Time/temperature control for safety food prepared from/mixed with ingredient(s) at ambient temperature not cooled to 41 degrees Fahrenheit within 4 hours. Observed steak being held at ambient temperature 48-50°F prepared at 3pm on 4/12/ per prep person who prepped meat.
