- comments: observed improper cold holding temperatures of tcs foods stored inside a prep cooler. The following were found at improper cold holding temperatures: sliced and wedges of tomatoes (47. 7f-48. 0f), chicken taquitos (48. 9f), salsa (48. 9f), two containers of shredded lettuce (48. 3f), two containers of raw chicken (48. 7-49. 4f), beef (48. 9f), two containers of shrimp (47. 7f-48. 9f). Must have cold holding foods at 41. 0f or below. Approximately 20lbs at $200 worth of product discarded. Priority 7-38-005, citation issued

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed improper temperature of prep cooler at 52. 0f with tcs foods stored inside for consumers consumption. A held for inspection tag has placed on unit. Please review comments for tag removal instructions. Must have cold holding units at 41. 0f or below. Priority 7-38-005, citation issued

35. Approved thawing methods used
- comments: observed a full, deep pan of raw beef thawing out at room temperature. Instructed to use proper cooling methods such as placing food item inside walk-in cooler overnight, or under cold running water.

39. Contamination prevented during food preparation, storage & display
- comments: observed a big pot of food and a container of food on the floor in the front prep area and inside the walk-in cooler. Instructed to refrain from storing food items on floor.

40. Personal cleanliness
- comments: observed female food handler in food prep area, preparing food with no hair restraint. Instructed all food handlers must wear hair restraints while in prep area.

41. Wiping cloths: properly used & stored
- comments: observed wiping cloths stored on cooking equipment throughout the front prep area. Instructed to remove and store inside sanitizer buckets and maintain.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no sink stoppers on site for three compartment sink. Instructed to provide and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed floors throughout facility in need of cleaning. Especially, under all the cooking equipment, in the walk-in cooler, and in the dishwashing area. Instructed to deep clean and maintain.

60. Previous core violation corrected
- comments: observed previous core violation from report #2617895, date: 05/21/2025, not corrected. #56: inadequate lighting in walk-in cooler, instructed to add more light fixture. Priority foundation violation 7-42-090 citation issued
