- comments: observed no person in charge with valid chicago food sanitation certificate. Instructed manager to designate person in charge during all hours of operation. Priority foundation

- comments: observed no valid chicago food sanitation certificate posted. Instructed manager to provide. Priority foundation 7-42-012

38. Insects, rodents, & animals not present
- comments: observed over 70 mice droppings scattered in several areas, on base of hot holding unit, on table under shake machine, on floor under cash register, inside fryer cabinets, on rolling carts, on prep tables, on shelving units, on top of paper bags in front storage and pop storage area, on pallets and shelving units, inside slicer and on bulk containers in rear storage area. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority 7-38-020(a)

- comments: observed foil lining the shelves throughout the facility in the front food service area. Instructed to remove all, clean and maintain as a means of preventing pest and insect harboring.

- comments: observed the walls and floors throughout the facility with a build up of dirt, grease and food debris under and around all equipment, under and around counters and shelving units. Instructed to clean and maintain all areas as a means of preventing pest and insect harboring.
