- comments: 2-102. 14(o) observed no employee health policy on site. Instructed to provide. Priority foundation 7-38-010.

5. Procedures for responding to vomiting and diarrheal events
- comments: 2-501. 11 observed no written procedure nor adequate supplies on site to properly respond to vomit and diarrhea events that can potentially occur inside of the facility. Instructed to provide. Priority foundation 7-38-005.

10. Adequate handwashing sinks properly supplied and accessible
- comments: 5-202. 12(a) observed no hot running water at the handwashing sink located in the rear food prep area. Instructed to repair and provide adequate hot running water (at least 100f) at all times. Priority violation. Citation issued 7-38-030(c).

- comments: 6-301. 14 observed hand washing signs are needed at all hand washing sinks located in the basement and 1st floor food prep areas and in the toilet rooms. Instructed to provide.

- comments: 4-602. 11(b-e) observed cutting boards with grooves and a chopping block with minor accumulated debris. Instructed to clean all.

- comments: 3-501. 16(a:1) observed cooked duck and cooked pork in the food prep area hanging in a unit at improper hot holding temperatures; observed about 20 lbs of cooked duck at 68f and about 10 lbs of cooked pork at 58f. Instructed manager to discard and denature said tcs foods and always maintain all hot tcs foods at 135f or above at all times. Employee discarded said tcs food during the inspection. Priority violation. Citation issued 7-38-005.

- comments: 3-501. 16(a:2,&b) observed time/temperature control for safety foods (tofu, rice etc) at an improper cold holding temperature located in the produce walk-in cooler; about 2 lbs of cooked rice at 70f, about 3 lbs of tofu at 59f, about 3 lbs of cooked chicken at 58f and about 2 lbs of cooked crab rangoon at 60f. Instructed manager to discard and denature all said tcs foods and always maintain all cold tcs foods at 41f or below at all times. Employee discarded said tcs foods during the inspection. Priority violation. Citation issued under violation #21 7-38-005.

- comments: 3-501. 17 observed no date labels for cooked tcs foods inside of the produce walk-in cooler. Instructed to provide date labels for all tcs foods that are cooked, cooled, and held for more than 24 hours inside of refrigeration units. The manager discarded said foods during the inspection. Employee discarded said tcs foods during the inspection. Priority foundation. Citation issued 7-38-005.

- comments: 3-501. 13 observed facility improperly thawing raw poultry in the 3 compartment sink and not under cold running water (not exceeding 70f). Instructed to always properly thaw all tcs foods under cold running water (not exceeding 70f) or in an adequate refrigeration unit at all times.

- comments: 3-302. 12 observed no labels on the dry storage containers located in the basement food prep area and on the spices containers located by the cooking equipment. Instructed to provide.

- comments: 6-501. 111 observed evidence of pest activity on site. Observed about 10 rodent droppings on the floor under the prep tables in the front food prep area and about 10 rodent droppings located on the floor in the corners of the additional front food prep area. Instructed to eliminate pest activity, clean and sanitize affected areas, and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 6-202. 13 observed the facility setting their pest control traps in the basement food prep area. Instructed to remove and only allow pest control to set all pest control traps.

- comments: 4-904. 11 observed tableware (chopsticks) being pre-set on the customer dining tables without adequate protection from possible contamination. Instructed to protect all tableware from possible contamination at all times.

- comments: 4-302. 13 observed no chemical test strips on-site to properly test the low-temperature dish machine and the 3 compartment sink. Instructed to provide. Priority foundation. Citation issued 7-38-005.

- comments: 4-601. 11(c) observed excessive accumulated food and grease debris inside, underneath, and around all cooking equipment. Observed excessive rust on the shelves located in both walk-in coolers located in the basement. Observed excessive dirt debris on the dish racks stored under the dish machine. Instructed to clean under around and inside all of the cooking equipment and replace shelves inside of the walk-in cooler and replace dish machine racks. Observed excessive accumulated food debris in the 3 door refrigerator located in the front food prep area and the exterior of the refrigeration units on the door handles. Instructed to clean and sanitize.

- comments: 5-204. 12 unable to locate the backflow prevention device on the ice machine located in the rear 1st floor food prep area. Instructed to locate or provide.

- comments: 5-501. 17 observed no covered receptacles inside of the women's toilet room. Instructed to provide.

- comments: 6-201. 13 observed unattached coving in both the women's and men's toilet rooms under the handwashing sink. Instructed to make tight-fitting.
