- comments: observed timed faucets not functioning properly in girls and boys washrooms. Instructed manager repair. Water must run continously for at least 15 seconds before reactivating faucet.

- comments: observed over 30 mice droppings scattered on pipe behind cooking equipment, 20 mice droppings scattered on floor under shelving units, along walls and corners in dry food storage area, 20 mice droppings scattered on floor along walls and corners in kitchen utility closet, 20 mice dropings scattered on floor along walls and corners in supply storage in lunchroom, 5 mice droppings scattered on rolling cart in kitchen. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a)

- comments: observed 1/4' gap on bottom of door #14 in lunchroom. Instructed manager to seal all outer openings on door to prevent pest entry.

- comments: observed milk crates used as shelving units. Instructed manager to remove all crates and provide adequate shelving for easy cleaning and pest control.

- comments: observed leaking faucet on 3-compartment sink and loose faucet on hand washing sinks in pre-k classroom. Instructed manager to repair and maintain.

- comments: observed peeling paint on ceiling in boys first floor washroom(west). Instructed manager to repair and maintain.
